Immunization / Physical Requirements
Penn-Trafford School District requires immunization requirements for the 2007-08 school year as a mandated regulation from the Commonwealth of Pennsylvania Department of Health. For more information, call 1-877-PA-health.

All students entering kindergarten or first grade and students entering sixth grade and eleventh grade are required to have a physical by a family doctor. Please bring a copy of your child's physical record to the nurse's office for review during the first week of school.

Required Immunizations
Students entering school for the first time in Kindergarten or first grade must have the following vaccines:

• 4 doses of tetanus and diphtheria3 doses of polio2 doses of measles (Rubeola)1 dose of German measles (Rubella)1 dose of mumps3 doses of hepatitis B or proven lab immunity
• 1 dose of varicella (chickenpox) vaccine or lab immunity, or written documentation of the disease.
